Training Overview Description

Course participants will be introduced to a standardized Clinic-based Care Coordination Curriculum and engage with working professionals interested in providing care coordination services within the medical home. Participants will develop and share valuable resources in communication, advocacy and the important role social determinants of health play in the care of patients and families. This course is designed for nurses, medical assistants, health aids, social workers, human service professionals, or someone with a passion for helping and supporting families.

The course was designed to facilitate a community of care coordinators to learn together and expand on current resources. This is an online course with 8 (eight) 90-minute weekly teleconferences, on-line classroom w/weekly assignments (readings, journal entries, and group discussion forums) outside of class followed by 2 (two) weeks for completion of final project.
